I,______________________________________________________, (herein after “Participant”) desire to volunteer my time and efforts at Miracle in the Desert, for Rock Bottom Restaurants,Inc.and its affiliates and subsidiaries including Rock Bottom Foundation (herein after referred to as “Company ”), and in so doing, agree to comply with all of the rules and regulations which may be established from time to time by Company. Participant understands that failure to do so may result in the Company disallowing me to act as a volunteer. Participation in this activity is by choice and wholly voluntary without pay or compensation of any kind and without liability of any nature on behalf of Company. All services will be performed at Participant’s own risk.
Participant acknowledges that he/she is at least 12 years of age and is fully aware of any risks that may be inherent in this particular event. If the Participant is a minor, individual signing below hereby consents as the parent or legal guardian of the Participant to all the terms and provisions contained herein.

Activity is diverse and could result in harm or injury to Participant, whether physical or mental, or damage to property from the participation in such Activities. Participant certifies that he/she is physically fit, has not been otherwise informed by a physician, and knows of no restrictions imposed on him/her that would in any way prevent them from actively participating in this activity. By virtue of participation, Participant authorizes Company, including any of its subsidiaries to use his/her name and/or likeness in tape and/or video for commercial and promotional use. Such production(s) may be used for local as well as national advertisement purposes. Company is given unconditional permission for the use of such photographic, video images or sound recordings in mediums including, but not limited to, websites, radio, newspapers, and company publications.

In consideration for being permitted to participate, Participant agrees to indemnify, defend, and hold harmless Company, its officers, directors, employees, agents, contractors, and invitees from and against all liability, demands, losses, damages (including consequential or punitive damages), fines, penalties, causes of action, claims, obligations, judgments, costs, or expenses (including without limitation attorneys fees and court costs) arising out of participation in this activity. Company shall have no responsibility whatsoever for any injuries and or grievances sustained as a result of the acts or failure to act of any person or entity, whether associated with Company or not, and arising out participation in the activity.

Participants are strictly prohibited from seeking out celebrity guests for purposes of autographs, photographs, or other. Any such attempts will result in immediate termination of volunteer activities including prompt removal from the premises.
